Patient Information

What to Bring

  • Driver’s license or photo ID
  • Health insurance card
  • Name and address of your primary care physician
  • Relevant x-rays and/or test results
  • Current medication list
  • Names and addresses of other physicians to whom we should send a report
  • Names and phone numbers of your preferred pharmacy and medical equipment company

Before your initial appointment, you will be asked to complete several forms. You can download the forms or we can mail them to you upon request. Please bring all completed forms with you to your appointment.

What to expect at your appointment:

After checking in with Reception, a nurse will record your height, weight, blood pressure and pulse. They will then review your medical history, asking questions about your current symptoms, medications and allergies, as well as your family, social, and occupational circumstances. In some instances, a respiratory therapist will test your lung function.

The physician will then review your records and interview you further about your sleep symptoms and concerns before performing a relevant physical exam. The physician will then determine if an overnight sleep study or other sleep testing is needed to further assess your condition and evaluate options for treatment. A report will be sent to your primary care provider as well as any additional physicians that you designate.

If you have an appointment with the sleep psychologist, you will be asked to fill out a questionnaire and complete tests that assist with understanding your sleep complaints. The sleep psychologist will meet with you, review this information, and use it as a starting point to investigate your insomnia complaints. The information collected will be used to develop a treatment plan, which may involve starting Cognitive Behavioral Therapy for Insomnia (CBT-I) and/ or a referral to a sleep physician.
