Gray top section: Minnesota Sleep Institute accepts most major insurance policies and participates in most HMOs and PPOs. We also accept Medicare and Medicaid. If your insurance policy requires a referral or prior authorization, please contact your primary care physician and/or your insurance company before your appointment. Co-pays are due at the time of service.
